*INSTRUCTIONS:* The McBurney Disability Resource Center scholarship application process involves the additional steps listed below. You must complete these to be considered for McBurney scholarships. McBurney staff are happy to assist you! * If you are NOT currently eligible for services through McBurney, do the following before the scholarship deadline: 1) Complete the McBurney Connect Online Student Application 2) Submit appropriate disability documentation. 3) Meet with your assigned access consultant, as requested, to determine scholarship eligibility, as well as eligibility for other requested accommodations. See McBurney's "Apply for Accommodations":https://mcburney.wisc.edu/apply-for-accommodations/ web page for detailed instructions. * Complete the "FAFSA":https://studentaid.ed.gov/sa/fafsa by the University’s *December 1 priority deadline*. International students should email a completed McBurney International Student – Financial Need Addendum to "heidi.johnson@wisc.edu":mailto:heidi.johnson@wisc.edu by the December 1 deadline. Download the form from "McBurney's scholarship web page":https://mcburney.wisc.edu/services/scholarships/. All but two of McBurney’s scholarships are scored heavily on financial need. Students will receive a financial need score of zero if they don’t complete this step by the December 1 priority deadline. * Submit a letter to the Office of Student Financial Aid if you want disability-related costs considered for your general UW-Madison financial aid application. For directions, see _Special Circumstances/Appeals_ on the "Office of Student Financial Aid’s web page":https://financialaid.wisc.edu/. (Click on _Applying_ in the top navigation. Then scroll down near the bottom.) * Ensure your two recommenders submit their recommendations by the scholarship deadline. You are responsible for monitoring adherence to the deadline. We prefer one recommendation come from a current or former instructor. Family, friends and McBurney staff are not eligible to be recommenders. * Upload a completed McBurney Center Scholarship Co-Curricular Involvement Form if you wish to be considered for the Ginsberg or WI Experience scholarship awards. These are the only two awards that don’t consider financial need. The form can be downloaded from the "McBurney scholarship web page":https://mcburney.wisc.edu/services/scholarships/. * Upload a transcript from the university where you received your last degree if you are an incoming transfer or a graduate/professional school student who did not receive your last degree from UW-Madison.
